Subject: 1st http load gets error

Here's a real firewall proxy related question for the firewalls list:

What would cause a first http connection to display:

        'Can't Access Document: http://www.ibm.com/. 
        Reason: Can't locate remote host: www.ibm.com. 

Clicking the 'reload' button or any dns cached location succeeds.
Could this be a dns problem, or a proxy problem?  I use the CERN 
httpd proxy with my fwtk software.
