advise needed on abuse policy

'lo list managers -

   I'm a sys admin for an Internet Service Provider.  We're in the
process of formulating a formal abuse policy (mail/news spams, warez,
personal email attacks. etc) and I'm hoping some other ISPs have already
done so and might be willing to provide some input.
