I sent mail to Cindy with the proper solution.  Since her pages are served
by BESTWWWD, the answer is to add one line to her "public_html/.bhtaccess"
file:
	UserRedirect / http://www.mydomain.com/

It tells the server to perform the redirect for any URL that resolves
to her directory and starts with "www.best.com/~".  This feature
appears to be unique to the BESTWWWD server.
