make sure you have both basic and NTLM authentication turned on in the
www service panel of the proxy server.

also, there is a bug in IE (including v4.0) that causes it not to
support user authentication when connecting to non-anonymous FTP sites
and SSL sites. according to ms, clients should use the winsock proxy
instead of the proxy server for those protocols to work correctly.
