What type of subscribe request:

	subscribe <listname>
or
	subscribe <listname> <address>

if the latter, try turning on munge_doamin. Chances are that the
address that is used as the identifier for the user (usually from the
from: header) is not the same as the address used in the subscription
request. If the former, try setting the list to auto subscribe and see
if it works. If so then there is something wrong with the subscription
validation procedure.
