That would depend heavily upon your mailer.

I can't see why UNIX would be a requirement, but who knows?  There must be
some non-UNIX mailer that allows piping to external programs.

It is.  I just hit the | key, type approve and hit return.  Or I did until
I bound the whole procedure to its own key.  I recommend consulting the
documentation for your mailer because this can save quite a bit of time.
