It's looking for a request to approve and can't find one. Perhaps you
could show us the file? Remember that it needs to have all it's headers,
as the Subject: header is scanned for the actions to take.

I've never used pine before, but I ran it, opened help and searched for
"pipe", which showed the proper key to push. (`|') That's the pipe, or
vertical bar, key.

I'm not exactly sure what it is you don't understand, so here's a try. Let
me know if this isn't what you needed:
Approval messages are sent by majordomo to listname-approval, who should
approve them (or not). No forwarding is involved. Whoever reads
listname-approval mail looks through the requests, finds one to approve,
and pipes it to approve by hitting the `|' key and typing "approve",
possibly using the full pathname ("/path/to/approve") if approve is not in
the normal path.
