Is your mailer inserting MIME headers around the body of the message
such that the Approved: header is no longer the first line? Also, you
don't want to leave a blank line between the Approved: header and the
bounced headers. Those headers that come back in the body of the bounced
message should be used for the approved outgoing message, but they'll be
missed if there's a blank line between the Approved: header and them.
