I hate to respond to my own query, but I found the solution and thought
I would share it for others who may have the same problem.

Somehow the uid/gid of the majordomo was not accepted by sendmail as a
trusted user.  This is really bizzare because the wrapper was installed by
root with suid bit set, and the installation passed the 'wrapper config-test'.

I simply added my majordomo uid to the sendmail.cf and now everything worked
as expected (e.g., added "Tmajordomo" to sendmail.cf)

PS.  The W_USER, W_GROUP and MAIL_GID were set correctly in the Makefile.
Oh well.  The change to sendmail.cf fixed the problem.
