I do have bulk_mailer installed and running for my larger lists.  But that
is still done with an "-outgoing" (as the example in the MD FAQ).  Nor
is there anything in the bulk_mailer documentation.

So, I would like to repeat my question.  Is there a reasonably simple
way of using bulk_mailer to avoid having to have an "-outgoin" alias?
If so, what?

Using bulk_mailer does prevent an EXPN on the outgoing, and the Majordomo
undocument "@filename" feature may help conceal the name of the -outgoing
list.  (although disabling EXPN as I have recently done is probably
better anyway).  But is there a way of not having to have the -outgoing at
all?
