Beware, I'm no expert and am responding from memory only, but ...
Duplicate mailings can be caused by sendmail initiating another instance
of 'clearing the mail queue' before the previous one is done.  This is most
likely to happen if there are problems delivering mail, i.e. the queue
cannot be cleared in the interval set by the sendmail -bq parameter
(AFAIK).  (not sure tho' if that wouldn't result in different "time stamps"?) 
Consequently, items are sent out eventually by all the active queue
processes.
