First, if you are running with "reply-to list" set you get much more
of this problem then you would otherwise.

Another thing you can do is when you see a posting that might start
a cascade, make use of the taboo_headers and taboo_body features
to force messages with certain things to go to the list manager.
I have used these features of majordomo several times for exactly
this purpose.  I don't know how bad the cascade would have been
if I hadn't been quick to use the taboo features.

Third, user education.  Put a pointer to a good document about user
email list behavior in your intro or info message.
