I see that you are using exim (from bits that I've removed).
exim can send a failure report for deliveries to pipes
which write the stdout or stderr is the option "return_output" is true
in the configuration for the pipe transport used to deliver the messages.
Also, you may wish to check-out the very new exim FAQ somewhere at
http://www.exim.org/ which has a still growing section on exim and
majordomo.
I could be entirely wrong about the nature of the problem, but when I see
an error report, exim, and appearently succesful delivery to a pipe this
is a good first guess.
