First, make sure you do not have some virtuser map redirecting all mail for that
domain to root. Second, double check your aliases making sure you did not leave
out a required quote, colon, or pipe symbol.
My 2 cents on the subject prefix is to leave it alone. I like it just the way it
is, and I can tell when mailing list messages are sent to me by the from: address
and other unique header lines which my MUA uses to automatically file this mail
into specific folders.
