URLs can be sent in email, and if a mail client is http savy, clicking
the URL will send an email reply, or at least complete an email window
waiting for the user to click "send". I am fairly certain this could
also be added to the confirmation message.

I have done this informally. I contacted email addresses that sent
subscribe messages, but never confirmed. Most of them did not realize
they were subscribing in the first place, so the lack of confirmation
allowed them to not subscribe. Some of them changed their mind about
subscribing by the time the confirmation message arrived, and only one
said they could not figure out the instructions.
I let it go at that, and later on was contacted by a user that wanted
to subscribe, could not figure out the confirmation reply, but was able
to write to majordomo-owner asking for assistance.
Personally, I would rather let a list maintain itself, and someone that
can not figure out a confirmation message, will probably be the one who
forgets how to unsubscribe later. (Even with instructions at the bottom
of every message posted to the list.)
I am getting on a soap box now, but I am only willing to help those who
are willing to help themselves. I disagree with the concept of providing
solutions to users who do not even know what the problem is. To me, that
is like trying to teach a first grade student jr. high algebra, and doing
so by giving them questions on one side of the page and answers on the
other.
