Yes it includes the attachment.  As far as Majordomo and your MTA 
(Sendmail or whatever) are concerned, it's all one big message.

That "40000" default for maxlength is the maximum message size in bytes. 
Decide on the largest attachment you're willing to allow to be sent to the 
list, convert to bytes, add a reasonable amount for message text, and use 
that as the maxlength setting in your <listname>.config.
