Subject: Operating system error

Whenever majordomo receives email my sendmail logs show the following error:
(domain names changed to protect the innocent)
to="|/usr/lib/MD/wrapper majordomo", ctladdr=<majordomo@thisdomain.org> 
(47/0), delay=00:00:20, xdelay=00:00:00, mailer=prog, pri=63312, dsn=4.0.0, 
stat=Operating system error

No other list creates this error.
Permissions for programs and files are as follows:

drwxr-xr-x   60 majordom daemon       8192 Jan 16 23:32 lists
-rwxr-xr-x    1 majordom smmsp       62511 Jan 17 07:40 majordomo
-rwsr-xr-x    1 root     daemon      16615 Jan 17 07:37 wrapper

Majordomo is set up as an alias in the /etc/mail/aliases file as:
majordomo: "|/usr/lib/MD/wrapper majordomo"
owner-majordomo: subcatcher@somedomain.net
majordomo-owner: subcatcher@somedomain.net

I am running Redhat 7.3 with sendmail 8.13.2.
smrsh is not running, nor was it compiled in sendmail.
Running demime as well, which is working flawlessly in the other lists.

Not being able to have majordomo receive mail properly is creating a burden 
as we have 42 lists with over 8000 users.
No maintenance is being allowed because of it.

If anyone has ANY idea, please let me know ASAP.
