majordomo digest guide

I've converted my "how to make a digest" guide into html at:

  http://www.concorde.com/~kevink/digest.html

and found a typo.  It occurs in two places.  One of the aliases,
"banjo-digest-outgoing" should be:

  banjo-digest-outgoing: :include:/path/to/lists/banjo-digest

and the digest aliases should also include an "-approval" alias.


Once 1.94 comes, I'll make any necessary updates, and I'm still
working on documentation about the digest command-line options
and the five configuration files.
