[mj2-cvs] subject_prefix

I'm not sure if this is a bug or a feature, but when I set subject_prefix to
[$LIST] the prefix is inserted repeatedly, even if it already appears in
the subject line.  If I set it to [list] (ie hardcode it) it's only inserted
if it's not already there.
